Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience License: Valid nursing license required, must have license in hand Certifications: CPR, BLS Must-Have: - Proficient in HomeCare HomeBase EMR and OASIS, with experience within the last 6 months - Reliable transportation, current driver's license, and required liability insurance - Willingness to travel up to 75 miles from home Description: The position requires providing, coordinating, and directing home nursing care based on physician orders. Nursing care must adhere to agency policies and procedures through the competent application of the nursing process. The successful candidate will be responsible to the Clinical Manager. Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses will be required. Home visits and periodic office meetings are part of the job responsibilities. An average of 5-10 patients will be assigned on a daily basis. Each type of visit and activity, including documentation and travel, holds a different point value. The contractor is expected to meet a productivity expectation of 30 points per week. Types of cases include Start of Care, Wound Care, Follow-up Visits, and more. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and patient care may result in cancellation of the contract. There are no guaranteed hours, and the contractor must accept every patient on their schedule regardless of travel distance. The successful candidate will need to manage various nursing tasks, ranging from admissions to medication assessment. Experience in adult home health care and hospice is preferred, as well as skills in case management and wound care. Candidates should have knowledge in infection control precautions and palliative symptom management. Strong clinical assessment, documentation skills, and ability to coordinate services are essential for this role.
