Key Responsibilities of the Registered Nurse - Home Health- Assessing, planning, implementing, and evaluating nursing support for well, chronic, or acutely ill individuals
- Coordinating communications and actions of various disciplines related to health and safety supports
- Ordering, reviewing, monitoring, and managing medications as prescribed
- Ensuring comprehensive health documentation and reporting when conditions change
- Supervising and training staff on health maintenance and disease prevention needs
Requirements of the Registered Nurse - Home Health- Valid AL or compact nursing license
- Proficiency with HCHB, OASIS, PointCare systems
- Experience in home health care and case management
