Responsibilities include performing initial in-home evaluations of patients. The evaluations will assess health status, strengths, care needs, and preferences. The evaluations will guide the development of individualized long-term care service plans. The nurse will review consumer medical documentation and health referral forms as relevant to the case. Evaluation data will need to be entered into electronic evaluation forms. The nurse must document any concerns, conflicting information, or other issues that arise during the evaluation process. Continuity of care must be emphasized to reduce fragmentation, duplication, and gaps in treatment plans. The nurse will discuss healthcare options, supports needed, service vendor options, and waiver options with consumers. Communication must be initiated between the Call Center and eligible consumers for enrollment into managed care health plans. Enhancing communication and collaborative relationships with interdisciplinary care team members is essential for improved care coordination and service delivery. Quality review data and required documentation must be collected to support outcome measurements and record case notes. Opportunities for health promotion and illness prevention should be identified. Maintaining knowledge of community resources, payor requirements, and network services is necessary for the target population. All standards established for this position must be met as outlined in the relevant performance criteria. Additional duties may be assigned by the Regional Nurse Manager or project management. All candidates must understand they will be conducting in-field assessments.
