Job Title: RN Case Manager Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficient in OASIS and HomeCare HomeBase EMR Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Requires home visits and periodic office meetings. Must have reliable transportation, a current driver’s license, and required liability insurance. Average number of patients per day: 5-8. Contractor will be assigned between 5-10 patients per day. Each visit and activity (documentation, travel, etc.) holds a different point value. Contractor must hit weekly productivity points of 30, equating to 40 hours per week. No guaranteed hours are offered. If contractor cannot meet financial needs due to low patient census, they should reach out to management to discuss floating to nearby locations. Types of cases include: Start of Care, Wound Care, Follow-up Visits, etc. First-timers are not accepted; recent specialty experience is required. Weekend and on-call requirements vary and should be discussed during the interview. Must be willing to travel up to 75 miles from the assigned care center. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted. Documentation must be completed within 24 hours of patient appointments. Delayed or incomplete documentation impacting billing and patient care may result in cancellation. All expectations must be understood and accepted prior to starting the role.
