Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 260 Days Shift: 5x5 Days Hours per Shift: Not specified Experience: Two years of experience in a home setting within the last fifteen years License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before the start date Must-Have: - Two years of experience in a home setting within the last ten years, which may include working in the field for a Certified Home Health Agency or Licensed Home Care Service Agency. - Conducting assessments that are similar to the UAS, such as OASIS. - Providing primary care to homebound persons. - Care management for individuals receiving home and community-based services. - Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of two patients per week. Description: Required licensure includes current RN licensure in Client. Experience should be related to conducting comprehensive assessments of patients. Nurses will be responsible for performing initial in-home pre-evaluations of patients. Evaluations will include assessing health status, strengths, care needs, and preferences. This guides the development of individualized long-term care service plans. Review of consumer medical documentation and relevant health referral forms is required. Data will need to be entered into electronic evaluation forms and transmitted as required. Document any concerns, conflicting information, or issues identified during the evaluation process. Emphasize continuity of care to reduce fragmentation, duplication, and gaps in the treatment plan. Discuss health care options and supports needed with consumers. Initiate communication with eligible consumers for enrollment into managed care health plans. Enhance communication and collaboration with interdisciplinary care team members to improve care coordination. Collect quality review data and required documentation to support outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain comprehensive knowledge of community resources and payor requirements for the target population. Meets all standards established for this position as outlined in the corresponding annual performance criteria. Performs other duties as assigned by the Regional Nurse Manager or other project management.
