Job Title: RN - Home Health Profession: Registered Nurse Specialty: Home Health Duration: 260 days Shift: 5x5 Days Hours per Shift: Not specified Experience: Two years of experience in a home setting within the last fifteen years License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Experience working in a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care - Experience caring for homebound persons or managing care for persons receiving home and community-based services Description: The RN will be responsible for performing initial home pre-evaluations of patients. This includes evaluating health status, strengths, care needs, and preferences to guide the development of individualized care service plans. The RN will review consumer medical documentation or health referral forms relevant to each case. Data from evaluations will be entered into an electronic evaluation form and transmitted as required. Any concerns or conflicting information discovered during the evaluation process must be documented. The RN will emphasize continuity of care to reduce fragmentation, duplication, and gaps in treatment plans. Health care options, supports needed, service vendor options, and waiver options will be discussed with consumers. Communication between the Call Center and eligible consumers for enrollment into a managed care health plan will be initiated. The RN will enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination. Quality review data and any required documentation to support outcome measurements must be collected and recorded in a CRM. Opportunities for health promotion and illness prevention will be identified. A comprehensive knowledge of community resources and payer requirements for the target population must be maintained. All established performance standards for this position must be met. Additional duties may be assigned by management as needed.
