Job Title: RN Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 5x5 Days Hours per Shift: Not specified Experience: Two (2) years of experience in a home setting within the last ten (10) years. License: Current RN licensure in Client. Certifications: BLS required before start date. Must-Have: - Nurses must have recent experience conducting comprehensive assessments of patients. - Experience working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Experience providing primary care to homebound persons. - Care management for persons receiving home and community-based services including Hospice and Assisted Living. - Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of 2 patients per week. Description: The RN will perform initial in-home evaluations of patients, which includes assessing health status, strengths, care needs, and preferences. The RN will guide the development of individualized long-term care service plans. The RN is responsible for reviewing consumer medical documentation and health referral forms relevant to the case. The RN will enter evaluation data into electronic evaluation forms and transmit as required. The RN will document any concerns, conflicting information, and other issues that surface during the evaluation process. The RN will emphasize continuity of care, reducing fragmentation, duplication, and gaps in treatment plans. The RN will discuss health care options, supports needed, service vendor options, and waiver options with consumers. The RN will initiate communication for enrollment into a managed care health plan. The RN will enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery. The RN is responsible for collecting quality review data and any required documentation to support outcome measurements and record case notes. The RN will identify opportunities for health promotion and illness prevention. The RN will maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population. The RN will meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template. The RN will perform other duties as assigned by the Regional Nurse Manager or project management.
