Description: Required licensure includes a current RN licensure in New York State. Candidates must have two (2) years of experience in a home setting within the last ten (10) years. This may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Conducting assessments similar to UAS, such as OASIS, is also required. Providing primary care to homebound persons and managing care for individuals receiving home and community-based services is essential. Experience as a paraprofessional, such as a Home Health Aide or Licensed Practical Nurse, caring for a minimum of two patients per week is also included. Preferred qualifications include recent experience conducting assessments in UAS-NY, especially in the last 365 days. Experience caring for a geriatric population and managing their chronic diseases, particularly in the home setting is desirable. Candidates should have a combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of two patients per week. Home care experience with only one patient, case management that is limited to utilization reviews, and assessments conducted telephonically do not satisfy the required qualifications. Candidates must submit a resume, and Nursys verification along with their NY license number. Ability to review health status questions and make evaluations based on UAS criteria for enrollment is necessary. Familiarity with technology in the workplace and knowledge of professional nursing principles and practices is important. Excellent organizational, interpersonal, written, and verbal communication skills are required. The role involves communicating effectively with multi-generational consumers and demonstrating sensitivity to the concerns of others. Comfort in a fast-paced, deadline-oriented work environment and the ability to handle complex tasks simultaneously is expected. Candidates should be able to perform initial in-home evaluations of patients, reviewing health status, strengths, care needs, and preferences to guide service plan development. Documenting concerns or conflicting information uncovered during evaluations is required. Candidates are expected to emphasize continuity of care while reducing fragmentation in treatment plans. Discussion with consumers about healthcare options and service vendor options is part of the role. Communication with eligible consumers for enrollment into a managed care health plan must be initiated. Enhancing communication and collaborative relationships with interdisciplinary care team members is necessary for care coordination. Collecting quality review data and documenting case notes into a CRM is required. Identifying opportunities for health promotion and illness prevention is part of the job. Maintaining knowledge of community resources and payor requirements for the target population is essential. Meeting all standards established for the position as outlined in performance criteria is necessary. Other duties may be assigned by project management.
