Description: The role requires a registered nurse with experience in home health. Candidates must have conducted assessments and delivered care in a home setting. Nurses are expected to perform initial evaluations of patients. This includes assessing health status, strengths, care needs, and preferences. Developing individualized long-term care service plans is a key responsibility. Reviewing consumer medical documentation and relevant health referral forms is required. Evaluation data must be entered into electronic forms and transmitted as needed. Documenting concerns or conflicting information during assessments is essential.
