Candidates must have two years of experience in a home setting within the last ten years. This experience may include working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Candidates should have experience conducting assessments similar to the UAS, such as OASIS. Experience providing primary care to homebound persons is also required. Care management experience for individuals receiving home and community-based services, including Hospice and Assisted Living, is preferred. Home care experience as a paraprofessional, such as a Home Health Aide or Licensed Practical Nurse, caring for a minimum of two patients per week is acceptable. Preferred qualifications include conducting assessments in UAS-NY, especially within the last 365 days. Ideal candidates should have experience managing chronic diseases in a geriatric population in a home setting. Home care experience as a paraprofessional or LPN combined with RN experience is acceptable. Other preferred qualifications that do not meet the required criteria include home care experience with only one patient, case management limited to utilization reviews for a health plan, and assessments conducted solely through telephonic communication. Successful candidates will have the ability to review health status questions and make evaluations based on UAS criteria. Candidates must be skilled in technology and possess an understanding of the regulations governing professional nursing practices. Excellent organizational, interpersonal, written, and verbal communication skills are essential. Candidates should demonstrate sensitivity to consumer concerns and be able to work effectively in a fast-paced, deadline-oriented environment. The role includes performing initial in-home evaluations of patients, reviewing consumer medical documentation, and entering evaluation data into electronic forms. Documentation of concerns during the evaluation process is necessary. Candidates will emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Discussion with consumers about healthcare options and necessary supports is an important aspect of this role. Candidates will initiate communication between enrollment entities and eligible consumers. This position requires enhancing communication and collaborative relationships with interdisciplinary care team members. Data collection for quality review and outcome measurement is expected. Maintaining knowledge of community resources, payer requirements, and network services is essential. All candidates must meet performance standards established for this position.
