Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: Mid Hours per Shift: Guaranteed hours not specified Experience: Two (2) years of experience in a home health setting within the last fifteen (15) years License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Ability to perform initial evaluations of patients, including health status, strengths, care needs, and preferences - Skills in technology and data entry Description: Urgent need for nurses in home health. Ability to review health status questions and make evaluations based on criteria for enrollment. Knowledge of the theory, principles, and practices of professional nursing. Familiarity with laws and regulations governing professional nursing practices. Excellent organizational, interpersonal, written, and verbal communication skills. Ability to communicate and work effectively with multi-generational consumers. Sensitivity to the concerns of others is essential. Comfortable working in a fast-paced, deadline-oriented work environment. Ability to successfully execute multiple complex tasks simultaneously. Strong capability to work independently. Perform initial in-home evaluations of patients to guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms as relevant. Document any concerns or conflicting information that arises during the evaluation process. Emphasize continuity of care to reduce gaps in treatment plans. Discuss health care options and necessary supports with consumers. Facilitate communication for enrollment into managed care health plans. Enhance communication with interdisciplinary care team members to improve care coordination. Collect quality review data and document case notes as required. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and payor requirements for the target population. Meet established performance standards for this position and perform other duties as assigned.
