License: Current RN Licensure in New York State Certifications: BLS certification required before the start date. Must-Have: Recent experience conducting comprehensive assessments of patients Description: Urgent need for qualified candidates. Candidates must be prepared to conduct hybrid Tele and in-field assessments. Ability to review health status questions and make evaluations and determinations based on defined criteria for enrollment is essential. Candidates must work effectively with technology in the workplace. Knowledge of nursing theory, principles, and practices is required. Understanding of state laws and regulations governing nursing practices is important. Excellent organizational, interpersonal, written, and verbal communication skills are needed. Ability to communicate and work effectively with multi-generational consumers is necessary. Candidates must demonstrate sensitivity to the concerns of others. Ability to perform comfortably in a fast-paced, deadline-oriented work environment is required. Candidates should possess the ability to successfully execute many complex tasks simultaneously. Strong ability to work independently is essential. Key responsibilities include performing initial in-home pre-evaluations of patients. Evaluations will include assessing health status, strengths, care needs, and preferences. Candidates will guide the development of individualized long-term care service plans. Review of consumer medical documentation and health referral forms is part of the role. Evaluation data must be entered into electronic systems as required. Documentation of concerns and any other issues identified during the evaluation process is expected. Continuity of care must be emphasized to reduce fragmentation and gaps in treatment plans. Discussion of health care options and service vendor choices with consumers is necessary. Initiating communication between consumers and relevant health plans is a key responsibility. Enhancing communication and collaborative relationships with interdisciplinary care team members is crucial. Collecting quality review data to support outcome measurements and maintaining case notes is required. Identification of opportunities for health promotion and illness prevention is expected. Comprehensive knowledge of community resources, payor requirements, and network services for the target population must be maintained. All performance standards established for this position must be met. Additional duties may be assigned as required by management.
