Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 8x5 Days Hours per Shift: 8 Experience: Two years in a home health setting within the last fifteen years License: Current RN Licensure in Client Certifications: BLS (Basic Life Support required before start date) Must-Have: Recent experience conducting comprehensive assessments of patients Description: This position is an urgent need for a Registered Nurse. Candidates must have the ability to review health status questions and make evaluations based on criteria for enrollment. Nurses should possess knowledge of the theory, principles, and practices of general professional nursing. Understanding state and agency laws and regulations governing nursing practice is essential. The role requires excellent organizational, interpersonal, written, and verbal communication skills. Candidates must be able to communicate effectively with a multi-generational consumer base. Demonstrating sensitivity to the concerns of others is required. The ability to work comfortably in a fast-paced, deadline-oriented environment is necessary. Candidates should successfully execute multiple complex tasks simultaneously and work independently. Job responsibilities include performing initial in-home pre-evaluations of patients. This evaluation will encompass the assessment of health status, strengths, care needs, and preferences. The evaluations will guide the development of individualized long-term care service plans. Candidates will review consumer medical documentation and health referral forms as relevant to the case. Data from evaluations must be entered into electronic evaluation forms and transmitted as required. It is necessary to document any concerns, conflicting information, or issues that arise during the evaluation process. It is important to emphasize continuity of care, reducing fragmentation, duplication, and gaps in treatment planning. There will be discussions with consumers regarding healthcare options, necessary supports, and service vendor options. Communication will be initiated between eligible consumers for enrollment into managed care health plans. Enhancing communication and collaborative relationships with interdisciplinary care team members is vital for improving care coordination. Quality review data must be collected, and required documentation for outcome measurements needs to be recorded. Identifying opportunities for health promotion and illness prevention is a key responsibility. Maintaining a comprehensive working knowledge of community resources and services for the target population is necessary. All standards established for this position must be met as outlined in performance criteria and bonus templates. Additional duties may be assigned by the Regional Nurse Manager or project management.
