Responsibilities:
- Conduct comprehensive assessments of home health patients, identifying physical, psychosocial, and environmental needs.
- Develop, implement, and evaluate individualized Plans of Care to ensure quality, continuity, and patient safety.
- Gather and incorporate data on patient factors influencing health status into the Plan of Care.
- Maintain accurate and up-to-date patient records, documenting all visits, contacts, and care plans.
- Adhere to infection control protocols to ensure safety for patients and staff.
- Educate patients and families on care procedures, medication management, and safety practices.
- Supervise and direct home health aides, LVNs, and STNAs to ensure quality service delivery.
- Assess and manage pain using appropriate techniques, educating patients and families.
- Coordinate referrals to community agencies and support services as needed.
- Monitor cases to ensure compliance with payer requirements.
- Provide on-call coverage and urgent care services as assigned.
- Communicate effectively with patients, families, healthcare team members, and providers.
- Attend interdisciplinary team meetings and contribute to patient care planning.
- Perform additional duties as assigned.
