Position Overview:
Role: Registered Nurse – Home Health
Location: Washington, DC
Start Date: 03/24/2026
Duration: 13 weeks with possible extension
Schedule: 5x8 Days (8:00 AM – 5:00 PM)
Call/Weekends: None required
Responsibilities:
- Perform Start of Care (SOC), OASIS-E admissions, recertifications, resumptions of care, and discharge assessments
- Complete accurate and timely documentation in HomeCare HomeBase (HCHB) Point of Care EMR
- Conduct comprehensive head-to-toe assessments and identify changes in patient condition
- Develop, implement, and update individualized care plans in collaboration with physicians
- Perform peripheral IV starts and manage IV therapy, including central lines and ports
- Administer medications, including intravenous infusions, and provide medication reconciliation and education
- Provide wound care, including wound VAC management and pressure ulcer staging and treatment
