Job title: Travel RN – Home Health
Location: Oklahoma City, OK
Assignment Length: 13 weeks with the possible extension
Projected Start Date: March 16, 2026
Shift: Days — 5x8 (8:00 AM–4:30 PM)
Guaranteed Hours: 40/week

Responsibilities:

Perform in‑home patient assessments and evaluate physical, emotional, and environmental needs.
Provide skilled nursing care including wound care, wound vac management, IV therapy, catheter care, and chronic disease management.
Develop and implement individualized patient care plans based on physician orders and patient needs.
Monitor patient condition, document changes, and communicate updates to the care team promptly.
Educate patients and families on disease processes, treatments, safety, and medication management.
Complete accurate and timely OASIS documentation for all assigned patients.
Document all visits, assessments, and interventions in EPIC EMR.
Coordinate care with physical therapy, occupational therapy, social work, and other disciplines.
