Job Title: RN Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: 1 year Home Health experience required License: Driver's License and proof of Insurance Certifications: BLS Must-Have: 1 year Home Health experience required Charting experience with Homecare Homebase required Description: Home Health Services are provided in various geographical areas. The typical caseload involves an average of 6-7 units per day and around 30 units per week. Admissions are weighted at 2.40 units, with an expectation of 2 admissions per day. Patient types include adults aged 18 and older, with common diagnoses such as wound care, wound vacs, pleurx drains, and diabetes management. Nurses will travel to multiple locations for patient visits. Required skills include charting experience, infection control practices, and critical thinking skills. Experience in catheter insertion and IV infusions is also necessary. Ability to drive at night is required. Support within the department is available from leadership and peers. Technology used includes EMR systems and mobile devices for documentation. Nurses are expected to schedule their own visits with patients the day prior. Cancelled appointments do not count as visit equivalents, and a minimum of 30 visit equivalents is required per week. A company phone will be provided for documentation and communication.
