Job Title: Weekend Field Nurse Profession: Nursing Specialty: Wound Care, Hospice RN, Home Health RN, Geriatric Duration: 13 Shift: Saturday & Sunday Hours per Shift: 7:00 am - 11:00 pm Experience: Minimum of 1 year License: RN Nursing Diploma or Degree from an accredited School of Nursing or University Certifications: Current CPR certification or willingness to obtain Must-Have: Bachelor’s degree preferred in the Science of Nursing. Minimum of one year of experience working with the frail, elderly, or long-term care population. Minimum of one year home care experience preferred. Minimum of one year wound care experience preferred. Description: The primary purpose of this position is to support the mission and core values while conducting participant assessments to determine appropriate levels of care. The role involves performing regular field visits to deliver essential nursing care, including wound care and health assessments. The nurse will coordinate with various stakeholders to ensure comprehensive participant support. Additionally, the nurse will document interactions and initiate emergency treatments as needed. Engagement in quality improvement initiatives is required, all within a participant-centered care framework. Adaptability to changing workloads is essential, along with effective communication with leadership. Responsibilities include performing participant assessments in person to determine appropriate levels of care according to the participant's plan of care. The nurse must respond promptly to calls from primary care providers and support staff. Documentation of all calls and contacts is necessary, along with follow-up as needed. Field visits will utilize full body assessment skills, critical thinking skills, and electronic medical records. Home visits will provide care and treatment as ordered, including wound care, IV hydration, lab draws, and nurse assessments. Education, instruction, and support will be provided to participants, families, and caregivers. Collaboration with various providers, staff, and suppliers is essential for comprehensive care. The nurse must initiate emergency treatment if necessary and document new orders when indicated. Appropriate documentation for incidents is required, along with adherence to quality measure policies. The nurse will actively participate in staff meetings and continuous quality improvement initiatives. Adjusting to changes in workload and schedules based on team priorities is necessary. Keeping leadership informed of any issues is essential. Other duties may be assigned as needed.
