The Home Care Coordinator serves as a discharge planning resource to hospital staff involved in home care planning.

The RN will set up home care services if needed and advise on appropriate community support organizations.
The coordinator functions as an on-site expert in home-based care.
The position requires availability on weekends, with potential shifts on Saturday and Sunday from 8:30 AM to 4 PM.
There are no holiday or call duties associated with this position.
The coordinator is expected to educate patients and their families regarding the discharge process and available resources.
