Provides care to these patients via the therapeutic use of self, the nursing process, the environment, instrumentation, and other health care team members. Expected to perform duties according to established standards of care and core values. Reports to the assigned Unit Director. Duties and Responsibilities: Performs assessment and data collection in an ongoing and systematic manner, focusing on the physiological, psychological, and cognitive status of patients. Analyzes assessment data to determine appropriate nursing diagnoses. Identifies individualized expected outcomes for patients. Formulates a goal-directed plan of care, prioritized based on determined nursing diagnoses and patient outcomes. Identifies patient and significant other learning needs and implements appropriate measures to meet these. Implements care in a knowledgeable, skillful, consistent, and continuous manner. Performs documentation duties which are timely, accurate, and concise.
