Duties and responsibilities include providing nursing care to all needing care in accordance with professional nursing standards and practices. Assessment involves assessing patients' physical, social, behavioral, and mental status through direct observation, interview, and medical record review. This includes assessment from history and data collection obtained from the patient and/or care providers. The nurse determines mobility, sensory deficits, and skin condition, adjusting the plan of care as appropriate. Documentation of a nursing assessment includes psychosocial, environmental factors, educational level, and needs. The nurse obtains and reviews available data and any data collected by other members of the health team. Identification of medical problems resulting from psychological or mental illness and use of medications is essential.
