Job Summary: The Care Manager - ED is responsible for providing comprehensive care coordination, utilization review, and discharge planning for patients in the emergency department. This role requires a professional with strong clinical expertise and computer literacy to ensure effective patient care and compliance with healthcare standards.
Required Qualifications:
- Five (5) years minimum of clinical experience as a Registered Nurse (RN).
- Experience with care coordination, utilization review, and discharge planning.
- Computer literacy.
Preferred Qualifications:Required Education:- Associates Degree (ASN, ADN) from an accredited nursing program.
Preferred Education:- Bachelor's or Master's degree in Nursing.
Required Certifications & Licensure:- RN licensed as a Registered Nurse in the State of New York.
- PRI and Screen certification within three months of employment.
- Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
- BLS certification.
Preferred Certifications & Licensure:- Case Management Certification.
