The role focuses on patient education, medication management, and clinical coordination within a pediatric and family practice setting, supporting a comprehensive approach to adolescent health.
Job Details
- Schedule: Days shift, 8 hours per day, 40 hours per week, Monday through Friday, 8:00 AM to 4:30 PM
- Required Qualifications: RN license, BLS certification, 2 years of pediatric or family practice experience. Flexible experience with pediatric emergency department, pediatric intensive care unit, pediatric acute care, and pediatric medical-surgical units is preferred due to the adolescent patient population.
- Responsibilities: Medication review, patient education, reviewing after visit summaries, managing electronic health record in-basket, coordinating care, and clinically managing patients and caregivers. Tasks include prescription refills, triaging calls, medication reconciliation, chart reviews, and collaborating with pharmacy, social work, and nutrition disciplines.
- Patient Population: Primary care for 10- to 26-year-olds, including well visits, sports and school physicals, men's and women's health exams (including pelvic exams and Pap smears), immunizations, acute sick visits, and confidential health services. Address school needs related to 504 plans and individualized education programs (IEP).
- Previous Employment: Candidates with prior work experience within any Johns Hopkins facilities must have at least one full year gap before reapplying as a traveler. Verification of prior employment in the healthcare system is required.
