Job Title: Case Manager Profession: Case Manager Specialty: Case Management Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 08:00 to 16:30 Experience: Minimum of 1 year case management experience in a hospital setting License: Required Certifications: Required Must-Have: - Experience with EMR systems - Strong organizational and communication skills - Ability to work collaboratively in a team Description: The Case Manager will work in an acute rehabilitation unit. This position will manage a caseload of approximately 14 patients. Candidates should possess a minimum of 1 year of case management experience in a hospital setting. Business casual attire is expected. This role does not require floating to other units. The work schedule is Monday through Friday from 8:00 AM to 4:30 PM. Healthcare experience is preferred.
