This position involves working within a case management setting.
The unit operates as a float unit and supports both psychiatric and rehabilitation services.
Candidates are expected to adapt to various units and provide comprehensive care to patients.
The patient-to-case manager ratio on night shifts is at least 25:1.
Familiarity with electronic medical records (EMR) systems, specifically Meditech and Midas, is essential.
Complex discharge planning skills are preferred for this role.
