Hospice Registered Nurse Case Manager Job Type: Full-Time Setting: Home Health / Hospice Work Location: Kansas City, MO (In person/Field Base) Pay Range: $85,000 - $90,000 per year Job Overview We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to provide high-quality hospice care to patients in their homes and senior living settings. This is a full-time, field-based nursing role where the RN will manage an assigned hospice patient caseload, coordinate care plans, support patients and families, and work closely with the interdisciplinary care team. The ideal candidate has prior hospice experience, strong clinical judgment, and is comfortable traveling throughout the day to visit patients in their homes. This position is with a well-established home health and hospice organization that has served patients for many years and offers long-term stability, strong benefits, and a supportive care environment. Key Responsibilities Provide skilled nursing care to hospice patients in their homes or senior living facilities Manage an assigned hospice patient caseload and coordinate individualized plans of care Perform patient assessments, monitor changes in condition, and provide appropriate interventions Educate patients and families on hospice care, symptom management, medications, and end-of-life support Collaborate with physicians, aides, social workers, chaplains, and other members of the interdisciplinary team Complete timely and accurate documentation using electronic charting Communicate regularly with patients, families, caregivers, and the care team regarding patient needs and changes in status Travel throughout the service area to complete daily patient visits Requirements Active Registered Nurse license in the state Prior hospice experience required Comfortable providing care in home-based and senior living settings Ability to manage a hospice caseload independently Strong communication, documentation, and patient education skills Must be willing and able to travel throughout the day to complete patient visits Must be a U.S. citizen or green card holder Benefits Competitive annual salary Health insurance Dental insurance Vision insurance Health Savings Account Flexible Spending Account 401(k) retirement plan Paid time off, starting at 3 weeks per year and increasing with tenure Short-term disability Life insurance Paid mileage Electronic charting Referral program Schedule and Travel Expectations This is a full-time position.
