We are seeking an experienced RN Case Manager to join a highly collaborative healthcare team in Colorado. This role is responsible for coordinating and managing patient care throughout the continuum, ensuring efficient utilization of resources, facilitating safe discharge planning, and promoting positive patient outcomes.
The ideal candidate will possess strong acute-care hospital case management experience and proficiency with the EPIC electronic medical record system. This position offers a desirable schedule of three 12-hour day shifts per week, providing excellent work-life balance with no on-call responsibilities.
Key Responsibilities
- Coordinate and manage patient care plans across the continuum of care.
- Assess patient needs and develop individualized discharge and transition plans.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams to optimize patient outcomes.
- Facilitate discharge planning, referrals, and post-acute care arrangements.
- Monitor utilization of healthcare resources and ensure compliance with regulatory requirements.
- Advocate for patients and families while providing education regarding care plans and available resources.
- Document patient assessments, interventions, and care coordination activities within EPIC.
- Assist with care transitions and continuity of care for both inpatient and outpatient populations.
- Participate in multidisciplinary rounds and case review meetings.
- Ensure timely communication with patients, families, providers, and community resources.
