Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Roles & Responsibilities
- Coordinate seamless care transitions for patients across the hospital
- Collaborate with physicians and social workers to create effective discharge plans
- Utilize Midas and Meditech systems to manage case documentation
- Advocate for patients while ensuring appropriate resource utilization
- Support complex discharge planning needs with creativity and compassion
Qualifications
- Active RN license in Florida or compact license
- At least 1 year of case management experience
- Hands-on experience with Midas and Meditech required
- Strong background in discharge planning preferred
Knowledge & Skills
- Proficient with Midas and Meditech electronic systems
- Skilled in complex discharge planning strategies
- Excellent communication and collaboration abilities
- Detail-oriented with strong organizational skills
