Location: Brooksville, Florida
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Coordinate seamless care transitions for patients across the continuum
- Collaborate with physicians, nurses, and social workers to create individualized discharge plans
- Advocate for patients and families to ensure they have the resources they need
- Utilize Meditech and Midas systems to track and document case management activities
- Help patients navigate complex healthcare systems with compassion and expertise
Qualifications
- Active RN license in Florida or compact license
- Minimum 1 year of acute care case management experience
- Strong organizational and communication skills
- Experience with Meditech and Midas preferred
- HCA experience is a plus
