Location: Must Reside in Virginia

Position: Nurse Case Manager II (RN)
Duration: 3+ Month Contract (Potential Extension)
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Telephonic & Field-Based Role
Mileage Reimbursement Provided
Key Responsibilities:
- Conduct comprehensive member assessments
- Develop and monitor care plans
- Coordinate care with providers and community resources
- Perform telephonic outreach and field visits
- Support members with health education and care coordination
- Document all activities in the case management platform
