Pay Rate:$40.00/ HR on W2
Shift Hours: M-F 8-5 CST
Job Summary
You will work as a field-based case manager helping elderly and disabled members who are receiving in-home care or nursing facility care.
You will:

Case load: 30–100 members

Assess patient needs
- Create care plans
- Coordinate medical and social services
- Work with doctors, families, and providers

Ensure members get proper, cost-effective care

Focus: high-risk members needing ongoing support
Location & Travel- Must live in Illinois
- Target counties:
- Cook
- Lake
- Will
- Kane
- Winnebago
- Rock Island
- Field role (50–75% travel) to member homes. Miles will be paid
- Remaining work is remote (home-based admin work)
Required License
Must have ONE of the following:
- RN (Registered Nurse) with active unrestricted IL license
OR - LCSW (Licensed Clinical Social Worker)
OR - LCPC (Licensed Clinical Professional Counselor)
Experience Needed
- 3–5 years clinical experience preferred
- 2–3 years experience in:
- Case management OR
- Discharge planning OR
- Home health coordination
Skills Required- Ability to work independently (very important)
- Good communication + organization skills
- Comfortable working from home
- Computer skills (MS Word, Excel, Outlook, PowerPoint)
- Ability to use multiple systems
Education
RN, LCSW, or LCPC with current unrestricted IL state licensure REQUIRED
