Job Title: Care Manager Nurse Profession: Nursing Specialty: Clinic, Care Manager, Ambulatory Care Duration: 13 weeks Shift: Monday to Friday - Days Hours per Shift: 40 Experience: Minimum 1 year of case management experience within the last 2 years License: Current full, unrestricted Registered Nursing license in Alaska required Certifications: Basic Life Support certification; National Case Management certification preferred Must-Have: Active Alaska nursing license at the time of submittal. Case management experience must be within the last 2 years. Proof of flu vaccine required. Description: This is a hybrid/blended role. Responsible for the active management of assigned patients. Will serve as a clinic nurse as assigned. Collaborates with medical providers and interdisciplinary team members to establish individualized medical goals. Aims to maximize patient healthcare outcomes. Coordinates multi-disciplinary care conferences for high-risk or complex patients referred into the case management process. Manages both acute and chronic care needs, as well as health maintenance. Continuously reviews electronic health record reminders and triggers. Provides outreach and education to patients. Participates in case management reviews where high-need patients are discussed. Works with a multi-disciplinary team as an advocate for the patient and their family. Fosters patient and family decision-making, independence, self-care, and growth. Utilizes problem-solving and anticipatory skills to meet the needs of patients and providers. Addresses patient needs in a prompt and efficient manner. Facilitates the coordination of patient referrals. Tracks the progress of referrals and care outcomes. Provides periodic status reports to providers on outstanding referrals. Serves as a primary link for other case managers and discharge coordinators. Ensures referral records are available for provider review. Proactively identifies delays and obstacles to care. Works to remedy individual cases and improve processes. Performs nursing duties that include assessment, documentation, education, and administering medications and therapies. Maintains annual competencies, including safety and infection control requirements. Performs duties in a responsible, respectful, and caring manner. Promotes effective nursing staff relationships. Maintains a clean and safe working environment, including adequate supplies and equipment upkeep. Reports defective or missing equipment and safety hazards. Adheres to infection control policies and procedures. Builds rapport and collaborates with team members across the organization. Participates in department and clinic team improvement efforts. Acts as an active member of the clinical team. Serves as a leader among nursing staff and unit coordinators. Participates in the development of the case management role in primary care. Responsible for participating in the triage role and responding to emergency situations. Performs other duties as assigned, including special projects necessary for comprehensive health services.
