Job Title: Inpatient Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 8 weeks Shift: Case Management - RN Hours per Shift: 8 hours Experience: Minimum of 2 years acute clinical nursing experience required. Minimum of 2 years case management experience required. License: Current state RN license. Certifications: Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred. Must-Have: Graduate of an accredited nursing school or college. BSN/MSN preferred. Knowledge of personal computer and software for word processing. Ability to enter and retrieve data from relevant computer systems. Knowledge of medical necessity guidelines and regulations. Ability to demonstrate caring, collaboration, competence, and communication behaviors. Description: The Inpatient Case Manager assesses, plans, implements, monitors, and evaluates options and services to develop an appropriate, individualized plan for patient care across the continuum of care. The Inpatient Case Manager will follow assigned patients, working with team members to prioritize and strategize daily activities. Using independent judgment and discretion, the patient must be assessed and reassessed as an ongoing process to address their broad spectrum of needs. The Inpatient Case Manager utilizes clinical knowledge, communication skills, problem solving, and conflict resolution techniques. Strong skills in assessment, organization, and time management are essential. The case management process requires critical thinking skills, customer service skills, setting appropriate goals, and measuring outcomes to ensure optimal patient outcomes. Consideration of financial and health plan resources is important. Works under the supervision of management to prepare and issue all disbursements with proper authorization in a timely manner. Ensures the accuracy of amounts to be paid and the proper coding. Reviews, matches, codes, and processes invoices. Researches problems and works with relevant personnel in a professional manner. Maintains all vendor accounts by reconciling statements monthly. Scans all incoming documents daily. Produces associated reports as needed. Demonstrates willingness to research, learn, and obtain knowledge for job performance. Demonstrates a courteous, professional demeanor and teamwork spirit. Utilizes critical thinking and applies sound clinical judgment and assessment skills for decision making. Physical demands include long hours of sitting and lifting up to 35 to 40 pounds.
