Facility Overview
Tuba City Regional Health Care Corporation (TCRHCC) is a Joint Commission-accredited healthcare facility serving a 4,400-square-mile region across the western Navajo and Hopi Reservations in Northwestern Arizona. The hospital provides comprehensive inpatient and outpatient services, including emergency medicine, surgery, orthopedics, OB/GYN, behavioral health, dental, ophthalmology, and specialty care services.

Position Summary
The RN Case Manager serves as the primary care coordination resource for identified high-risk patients in both inpatient and outpatient settings. This role focuses on specialty clinic populations requiring dedicated case management services and ensures patients receive coordinated, quality, and cost-effective healthcare throughout the continuum of care.
The Case Manager facilitates care transitions from admission through discharge, coordinates referrals and community resources, performs utilization review activities, and collaborates with interdisciplinary teams to support positive patient outcomes. The position also assists patients experiencing social, behavioral, and mental health challenges while helping them navigate healthcare and community resources.
The RN Case Manager works with culturally diverse patient populations and addresses barriers to care including chronic illness, mental health concerns, substance abuse, domestic violence, illiteracy, and limited access to healthcare resources.
Essential Responsibilities
Case Management & Care Coordination
- Assess patient needs and develop individualized care plans.
- Coordinate healthcare services across inpatient, outpatient, and specialty care settings.
- Facilitate transitions of care from admission through discharge.
- Coordinate referrals to community resources, continuing care agencies, and specialty providers.
- Monitor patient progress and modify care plans as needed.
Utilization Review & Discharge Planning
- Evaluate patients for medical necessity and appropriate level of care.
- Collaborate with physicians and advanced practice providers regarding admission status and discharge planning.
- Perform utilization review activities to ensure effective resource utilization.
- Develop safe discharge plans and coordinate post-discharge services.
Emergency Department Support
- Assess patients presenting to the Emergency Department for care coordination needs.
- Identify patients requiring case management intervention.
- Assist with discharge planning and outpatient follow-up arrangements.
Patient Advocacy
- Serve as an advocate for patients and families.
- Assist patients in accessing healthcare, behavioral health, and community resources.
- Support individuals experiencing mental health challenges, chronic disease, substance abuse, and other complex care needs.
- Promote continuity of care and improved patient outcomes.
Interdisciplinary Collaboration
- Work closely with physicians, nurses, social workers, behavioral health providers, and ancillary departments.
- Participate in care planning meetings and interdisciplinary rounds.
- Provide guidance regarding transitional care and regulatory requirements.
