Job Description:
Join a leading healthcare team as an experienced RN Case Manager, coordinating patient care, discharge planning, and utilization review. You'll collaborate across departments to ensure safe, efficient, and patient-centered transitions of care in an acute hospital setting.
Requirements:
Experience: 3–4 years recent case management, utilization, or discharge planning (teaching hospital preferred)
Certs: BLS, BSN, and active CA RN license
Skills: InterQual, care coordination, concurrent review, and discharge planning
Schedule: 4x10s (Mon–Fri, variable day off) | No weekends
Start Date: 12/01/2025
