The primary patient population includes those with chronic disease management such as CHF, COPD, and DM. The caseload is approximately 6 visits per day, with adjustments for Oasis visits. The RN will be required to schedule their active patients and will have support available from management and home health aides when needed. Technology used includes EMR systems, specifically EPIC. The RN will receive necessary equipment such as a laptop, cell phone, and trunk stock for patient visits. The position requires participation in weekend rotations, working every third weekend, but no on-call duties. The RN should be prepared to participate in morning meetings remotely and attend team meetings in person every other month.
