Helps to assesses post-heart transplant graft function and general health status and collaboratively determines need for additional assessment or intervention. Coordinates any changes to medication regimen or other regimens with attending MD and /or APP and Pharm D. Frequent laboratory results oversight to detect changes that may need immediate action. Communicate with multidisciplinary team members, patients and families in a timely manner in regard to patient care issues. Facilitates patient activities with consulting departments and physicians. Assists other nurses and staff to assess, plan, implement and evaluate care of the transplant patient.
