Summary: The Registered Nurse Case Manager is responsible for coordinating and supporting patient care throughout the healthcare continuum to promote efficient resource utilization and positive patient outcomes. This role involves evaluating patient needs, creating detailed care strategies, and working closely with interdisciplinary teams to deliver high-quality, cost-conscious care. Key Responsibilities: Evaluate patients’ clinical, psychosocial, and functional status Partner with a multidisciplinary team—including physicians, therapists, social workers, and fellow case managers—to design patient-focused care plans Create and revise individualized care plans in alignment with clinical standards and patient goals Oversee care transitions to ensure continuity and access to appropriate services and resources Serve as a communication link between patients, families, and healthcare team members Track patient progress and modify care plans based on outcomes and changing needs Maintain adherence to regulatory requirements and payer guidelines Support patient advocacy by addressing obstacles to care and promoting fair access to services Perform utilization reviews and monitor length of stay to ensure compliance with best-practice guidelines Provide education to patients and families regarding diagnoses and available community support Accurately document all case management activities in the medical record Participate in care team meetings and collaborative discussions Complete additional duties within the scope of the role as assigned Follow all safety procedures, infection prevention measures, and regulatory policies Required Skills and Qualifications: Strong analytical thinking, customer service orientation, and effective communication skills Proficiency in reading, writing, and speaking English Ability to understand written materials, follow verbal directions, and use basic computer systems Physical capability to sit or stand for extended periods, move throughout the facility, and handle or transport equipment weighing up to 50–100 lbs, including pushing, pulling, and bending Ability to collaborate professionally with diverse staff while maintaining a positive and respectful demeanor Fine motor skills and coordination necessary for handling equipment and assisting with procedures Capacity to manage frequent interaction with patients and the public while meeting deadlines in a fast-paced environment Ability to work both independently and with minimal supervision when needed Flexibility to handle occasional schedule changes and maintain attention to detail over prolonged periods Adequate vision and communication ability to safely and effectively perform job duties
