Position Overview:

We are seeking a highly skilled Registered Nurse (RN) Case Manager with strong Utilization Review and InterQual experience to join our care management team. This role plays a vital part in ensuring patients receive the most appropriate level of care while optimizing hospital resources and supporting quality outcomes.

The ideal candidate will bring a deep understanding of acute care processes, demonstrate proficiency in EPIC EMR, and possess the ability to assess patient needs, coordinate interdisciplinary care, and apply evidence-based utilization criteria. This position offers an opportunity to contribute directly to patient advocacy, care coordination, and healthcare efficiency in a dynamic hospital environment.

Key Responsibilities:
  • Perform initial and concurrent patient assessments to determine care needs and discharge readiness.

  • Utilize InterQual criteria to conduct comprehensive Utilization Reviews ensuring medical necessity and compliance with payer requirements.

  • Collaborate effectively with physicians, nurses, social workers, and other healthcare professionals to develop and implement individualized care plans.

  • Document all case management activities accurately and promptly within EPIC EMR.

  • Participate in multidisciplinary rounds and discharge planning discussions to ensure safe, timely transitions of care.

  • Maintain compliance with hospital standards, regulatory guidelines, and best practices in utilization management.

  • Provide coverage during holidays as required.
