- Manage complex cases involving chronic illnesses such as:
- Diabetes
- Cardiovascular conditions
- Behavioral health issues
- Conduct utilization management (UM) and coordinate care plans.
- Collaborate with healthcare providers, internal teams, and external partners to ensure quality, cost‑effective care.
- Support patient education, care transitions, and resource utilization.
- Maintain documentation, follow compliance standards, and meet workflow timelines.
