We are currently seeking an experienced Registered Nurse (RN) – Case Management / Care Coordination for a 13-week travel assignment at a leading acute care hospital in Carmichael, California. This opportunity is ideal for nurses with strong case management experience who are skilled in discharge planning, utilization review, care coordination, and collaborating with multidisciplinary teams to ensure optimal patient outcomes.

Position Details
  • Position: Registered Nurse (RN) – Case Management / Care Coordination
  • Location: Carmichael, CA
  • Setting: Acute Care Hospital
  • Shift: Days
  • Schedule: 8-Hour Shifts (Typically Between 8:00 AM – 5:00 PM)
  • Guaranteed Hours: 40 Hours Per Week
  • Contract Length: 13 Weeks
  • Start Date: July 7, 2026
  • Weekend Requirement: Every Other Weekend (Minimum One Full Weekend Per Month)
  • Floating: First to Float
  • EMR System: Cerner / InterQual
Responsibilities
  • Complete comprehensive patient assessments and develop individualized discharge plans.
  • Coordinate patient care across multiple disciplines and healthcare settings.
  • Perform utilization management and utilization review activities.
  • Apply InterQual criteria to support medical necessity determinations.
  • Collaborate with physicians, nursing staff, social workers, and community resources.
  • Facilitate safe and timely patient discharges.
  • Assist patients with complex discharge needs, including uninsured and homeless populations.
  • Coordinate post-acute care services and resource referrals.
  • Monitor patient progression throughout hospitalization and address barriers to discharge.
  • Maintain accurate and timely documentation within the EMR.
  • Communicate effectively with patients, families, and interdisciplinary teams.
Preferred Qualifications
  • Experience working with diverse patient populations, including uninsured and homeless patients
  • Familiarity with multiple payer sources and insurance authorization processes
  • Strong interdisciplinary teamwork and problem-solving skills
