Job Title: Case Management Registered Nurse (RN) – Day Shift
Location: 975 South Fairmont Ave, Lodi, CA 95240
Job Overview:
We are seeking a dedicated and experienced Case Management Registered Nurse (RN) to join our team. This role involves coordinating patient care, ensuring effective treatment plans, and supporting optimal patient outcomes in an acute care setting.
Shift Details:
- Day Shift: 8:00 AM – 6:30 PM
- 40 hours per week
- Days per week: Variable (including weekends)
- No on-call requirement
Key Responsibilities:
- Coordinate and manage patient care plans
- Collaborate with healthcare teams to ensure continuity of care
- Assess patient needs and develop discharge plans
- Utilize electronic medical records effectively (Cerner system)
- Ensure compliance with healthcare standards and regulations
Preferred Qualifications:
- Bachelor’s Degree in Nursing (BSN)
- Experience in a care management role
- Case Management Certification
