We are seeking a Registered Nurse – Case Manager to coordinate and manage patient care throughout hospitalization and post-discharge. The RN will assess patient needs, develop and implement care plans, collaborate with physicians, nurses, and social workers, ensure appropriate utilization of resources, and provide patient and family education to promote positive health outcomes.
