- Day Shift: 5x8 hours
- Hours:08:00 AM – 05:00 PM
- Schedule: Every other weekend required
- Float: May float to other UCSF facilities (within 75+ mile radius)
- Manage pediatric patients’ care plans from admission through discharge
- Coordinate care with interdisciplinary teams (physicians, therapists, social workers, etc.)
- Perform discharge planning and arrange post‑acute services
- Conduct utilization review and ensure appropriate level of care
- Work with families on education, resources, and transition of care
- Navigate complex cases in NICU/PICU and general pediatrics
- Document all case management activities in the hospital’s charting system
- Ensure safe, timely, and effective patient transitions
