Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years of current experience in an acute care adult inpatient hospital setting. License: Current RN license required. Certifications: BLS required; Case Management Certification and BSN preferred. Must-Have: Current/recent experience in an acute care adult inpatient hospital setting. Candidates must float within the hospital based on census. Documentation experience using EPIC required. Weekend and holiday commitment is necessary, with every other weekend required. Description: The Case Manager RN will be responsible for coordinating patient care and ensuring appropriate resource utilization. The role involves assessing patient needs and developing care plans to facilitate optimal health outcomes. Candidates will engage with interdisciplinary teams to provide high-quality care and support to patients and families. Experience in acute hospital settings, appeals and denials, care coordination, concurrent review, and continued stay reviews is essential. The expected daily patient census is approximately 20. The candidate must be adaptable to changing environments and ready to support various patient diagnoses, including medical, surgical, cardiac, vascular, and some neuro-related conditions. Attention to detail and strong communication skills are required to effectively manage patient care and documentation.
