Responsibilities include assessing patient needs, developing and implementing individualized care plans, coordinating with interdisciplinary teams, facilitating discharge planning, and providing patient and family education. The RN Case Manager also monitors quality metrics, documents patient care, and ensures compliance with regulatory standards.
