Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 16 weeks Shift: Monday-Friday Hours per Shift: 8 hours (Approximately 0800-1630) Experience: Minimum of 3 years as a Registered Nurse; 3+ years of case management experience preferred License: Required Certifications: BLS (AHA) Required Must-Have: Inpatient Acute Care Hospital Case Management Experience EPIC Experience Required 5+ years of RN experience in an acute care setting BSN preferred Certification in Case Management preferred Description: The RN Case Manager facilitates discharge coordination. They promote effective patient throughput in the inpatient setting. The role involves ensuring continuity of care for specified populations in an acute care environment. This includes collaboration with patients, surrogates, families, physicians, nurses, and other members of the healthcare team. They address patient needs through effective coordination of services. The RN Case Manager evaluates clinical requirements. They synthesize patient goals of care, treatment preferences, and available resources. In developing discharge plans, they account for continuum of care needs. The plans respect the patient's right to self-determination. The RN Case Manager will manage a caseload independently within 1-2 weeks of training. Weekends and holidays are not required, but holiday coverage is appreciated. No call is required for this position.
