Job Title: URMHC Level 2 Registered Nurse Case Manager

Location: New York, NY

Pay: $2801.20 weekly

Shifts: Monday to Friday, 8:00am-4:30pm, weekends per need

Seven Healthcare are seeking a dedicated URMHC Level 2 Registered Nurse Case Manager specializing in home health care to join our dynamic team in New York. This travel assignment offers a supportive work environment, focusing on providing expert case management services.

Key Responsibilities of the URMHC Level 2 Registered Nurse Case Manager
  • Develops and revises treatment and discharge plans with the patient, family, and care delivery team.
  • Completes OASIS assessments, reassessments, and related documentation in a timely manner.
  • Notifies physicians of patient needs and changes in condition, and completes necessary orders and summaries.
  • Ensures patient privacy and confidentiality are respected.
  • Initiates preventive and rehabilitative strategies.
  • Provides specialized nursing services as needed.
  • Educates the patient, family, and team members on disease processes and preventive strategies.
  • Coordinates and supervises care provided by the Home Health Agency.
  • Interacts professionally with patients, physicians, and referral sources.
  • Ensures compliance with clinical records and documentation requirements.
  • Maintains knowledge of available community services.
  • Visits patients based on need rather than geography.
  • Attends patient care conferences as required.

Requirements of the URMHC Level 2 Registered Nurse Case Manager
  • RN License (NY License ONLY)
  • One year of clinical nurse experience in a hospital setting
  • 9 months of home care experience preferred
  • BLS certification required
  • Certified Home Health Care Experience
  • Experience with EPIC or Netsmart preferred
  • OASIS and Telemetry Experience
  • Critical thinking and effective clinical judgment
  • Strong communication, assessment, and relationship-building skills
  • Current NYS driver's license
