They will be responsible for discharge planning and care coordination for patients. The role requires knowledge of admission criteria and determining medical necessity. Candidates should be familiar with evidence-based guidelines and CMS regulations. Charting and documentation will be performed using the Epic system. Weekend shifts will be required every other weekend or 4 days within a 4-week schedule. The clinician may float within the case management services to meet facility needs. Preferred certifications include ACM certification and experience with acute care settings. Patient populations will include neonates, adolescents, adults, and geriatrics. The daily census will involve managing multiple patient cases effectively. The Case Manager RN will work closely with interdisciplinary teams and support services. Communication and collaboration with healthcare providers are essential in this role. The position participates in pre-cert reviews and prior authorizations as needed. Knowledge of ICD-10 coding and HIPAA guidelines will be beneficial. The candidate must be prepared to work two out of the three major holidays: Thanksgiving, Christmas, and New Year. Flexibility and adaptability in scheduling are key attributes for success in this role.
