Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum Years of Experience: 1 License: Valid State License required Certifications: All required certifications must be in-hand when applying. Crisis Prevention Institute Blue Card is required. Must-Have: Epic experience must be noted in work history. Two Supervisor references required. Description: This position primarily involves inpatient case management. There may be occasional cross-coverage for discharge planning and admission utilization management. The schedule is Sunday to Thursday, with some flexibility required. Charge experience is valuable for this role. The candidate must have experience with patient charting systems. Experience in care coordination and discharge planning is essential. Knowledge of Medicare and Medicaid services is necessary. Familiarity with HIPAA guidelines is required. The role includes conducting needs assessments and managing prior authorizations. Interdisciplinary support and collaboration with social services will be part of daily responsibilities. The candidate will work with a diverse patient population, including adolescents, adults, and geriatrics. Daily tasks may involve managing admission criteria and planning patient care pathways. All required certifications must be verified. A CPI Blue Card from the Crisis Prevention Institute is mandatory for this position. All candidates must adhere to the necessary compliance and background check standards. Maximum 7 days of requested time off per 13-week contract is preferred.
